The Wellness Coordinator assesses the health, functional and psycho-social status of residents, initiates individualized service plans and proactively manages care and services for each resident in collaboration with the care team and any third- party vendors.  The Wellness Coordinator is responsible for evaluation of effectiveness and maximization of the resident’s opportunity to remain in the assisted living environment. The Wellness Coordinator oversees and promotes the residents’ health and wellness through regular visits with each resident, resident assessment, and ongoing communication with the resident, resident’s family and the care team. The Wellness Coordinator is directly responsible to the Executive Director and indirectly responsible to the Regional Nurse Manager.



  1. Ensures timely completion of resident assessments and service plans related to function and physical status at pre-admission, admission, 30 days, annually, and with change of condition or at intervals specified by state regulations.
  2. Using information gathered during the assessment process, develops the service plan, communicates the service plan to the resident, Proxy, and other relevant members of the health care team.
  3. Assesses individual residents to determine ability for self-administration of medication.
  4. Assess/evaluate potential residents for assisted living level of care. This may involve assessing/evaluating potential residents in their home or another healthcare setting; must be able to drive to location and perform assessment/evaluation professionally and timely.
  5. Development and oversight of health promotion and disease prevention programs that include administration of immunizations as appropriate, and adherence to policies and protocols for infectious disease management such as flu outbreaks and C-difficile.
  6. Promotes and assists the residents to maintain their maximum physical, mental and psycho-social function and to reduce risk of infection, trauma and mistreatment.
  7. Provides ongoing in-service for all staff as relevant to the needs of the residents and the community.
  8. Coordinates, along with the Executive Director, family/resident care conferences and serve as a direct liaison between residents, families, and staff. Work with residents, families and other staff to ensure that residents are at the appropriate levels of care.
  9. Ensures that all regulatory time frames for review of assessments and service plans are met.
  10. Works collaboratively with other community team, including the Executive Director to ensure compliance with all state regulatory requirements including mandatory reporting to State agencies. Ensure that all incident reports are completed, filed and that follow-up is conducted appropriately.
  11. Participates actively in the Primrose community’s quality improvement process, including regular attendance at, and reporting to the Quality Committee. Participates in ongoing facility efforts to improve the overall quality of the nursing care, including efforts to evaluate and address the causes of various care-related problems and deficiencies.
  1. Maintains confidentiality within the workplace with all written and verbal communications pertaining to residents, staff, and community operations.

Job Requirements


Knowledge of current state laws and regulations that apply to the practice of nursing in an assisted living setting.  Has strong organizational and time management skills and the ability to prioritize responsibilities.  Demonstrates leadership skills in principles of team building, communication and motivation of others. Knowledge of Alzheimer’s Disease, other dementias and related memory impairments is preferred.  Basic knowledge of Microsoft Outlook and Word.  Able to work primarily day hours, Monday through Friday.



Current, active registered nursing license in the state the community is located in.  Maintain CPR and First Aid Certification per state regulations.  Meet annual continuing education hours required for nursing license if applicable. Experience in a long-term care, assisted living, or case-management is preferred.
